Sphenoid sinusitis explanation

Question:

I have been having severe headaches and swollen glands/lymph nodes for some time. I am now having trouble at times with my speech and my vision has changed quite a bit. I am having neck and arm pain as well. I finally convinced my PCP to order an MRI (unfortunately done of the sinuses only) This has been going on for many years with symptoms and pain getting worse and worse, could you please explain the diagnosis/prognosis? Thank you immensely. Here is what the report says: IMPRESSION: 1.Mucosal thickening within the left side of the sphenoid sinus suggest sphenoid sinusitis.

2.9 mm nodular area of mucosal thickening within the floor of the right maxillary sinus could be due to a retention cyst or polyp.

3.Nasal septal deviation to the left with large right middle turbinate concha bullosa. There may also be a very tiny left middle turbinate concha bullosa present.

4.Large bilateral Haller cells markedly narrow the maxillary infundibula bilaterally

Answer:

Of the four diagnostic points you list, 2 - 4 may predispose you to recurrent sinus infections or contribute to breathing problems in your nose, but are otherwise of no consequence.  The first, ie sphenoid sinusitis, can cause headaches and in extreme cases, can be associated with visual changes, but your report does not really suggest such a severe infection.  In addition, a severe infection is not likely to go on for years without causing further problems.   

If you have been experiencing speech and vision problems along with a varied distribution of pain, I suggest a full neurological evaluation.  I would consider seeing a neurologist initially for further evaluation.
