Ischemic Lesions / Encephalitis

Question:

My father had metastatic prostate cancer that he was taking hormone treatment and responding well. His PSA came down from 74-35-6 but three hours after he took his second shot he had speech impairement and four days after started loosing his consciousness and he remains unconscious. They thought it could be a stroke but the first MRI did not show anything and the second MRI showed teh following: there is evidence of extensive bilateral tempro insular & to lesser degree fronto parietal abnormal signal intensity on FLAIR and T1 involving mainly the corte & the subcortical white matter. On the left side, teh central gray matter mainly the globus pallidus,putamen & caudate nucleus are involved by this process.The ventricular system is not enlarged, Faint abnormal signal intensity on T2 and FLAIR of the parhypocampal gyri could be noted on both sides. Multiple focal lacunar abnormal signal intensities on T2 & FLAIR of the periventricular and subcortical white matter suggestive of deep white matter infarts.No infra tentorial abnormal signal intensity is seen. The findings is suggestive of Herpetic Encephalitis.Where the CT scan showed that the ventricular system is not enlarged. Large extensive bilateral fronto-temporo insular and to lesser degree parietal hypodense areas are seen on both sides exerting moderate mass over the ventricular syste. No evidence of intra or extra axial haematoma. What could the problem be in your opinion. Thanks in advance
