Back Pain and Maybe Displacement of Vertebrae

Question:

I was in a high speed impact about 18 months ago, and ever since then my lower back pain has worsened. The vertebrae are not aligned ans feels like there is 2 pieces of bones “floating” around. It makes a crunch like feeling when any pressure is applied. I have T3s for the pain, but it is not getting any better, only worse. There is swelling as well around the area. If I apply heat, the pain subsides a little. I sleep with a pillow under my knees which alleviates it a bit too. Do I need back surgery or is there a different way of fixing this? Thanks
