Patient Refusal to Dental X-Ray

Question:

If a patient keeps on for years refusing dental X-ray and the dentist continues to treat that patient, would that dentist be liable in case of later hidden damages showed up. Would the dentist be liable for negligence? and what if the patient signed his refusal to x-ray, would the dentist still be liable for negligence?
